Please print out the form, fill out, and then mail the form and check back to CyRide at the address below.

Make check for the exact amount payable to CyRide. Please do not mail cash.

Please note, Iowa State University (ISU) Students with a current ISUCard do not need a bus pass or tickets. Students just show their student ISUCard and ride free as they've paid tuition to Iowa State which includes fees to Student Government who funds CyRide.
